Medical records are confidential. We will not release information about your care to anyone, including other family members, without a signed release. Please allow up to a week to process these requests.
Financial Information & Insurance
We require payment of your deductible, co-payment, and past-due account balances at the time of service.
We accept cash, checks, debit and all major credit cards.
We will submit claims to most major insurance carriers. Please bring your insurance card with you to every visit so we can ensure that our records are accurate.
We participate with many insurance plans. We recommend that you call your insurance company to verify that we participate with your plan.
Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs.
Many insurance companies require a referral for a visit to a specialist's office. Please make sure to contact your insurance company and/or your primary care physician to ensure that you do get a referral if one is required. Failure to obtain a referral, when necessary, may cause your insurance company to deny your claim.
If prior authorization for testing or surgery is needed, our office will obtain this from your insurance company.

All prescription and refill requests should be requested during normal office hours. There is a 24 hour notice on prescription refills. Please have your pharmacy fax the office at (850) 784-1975.
